- Clinical trial
In 3649 patients with high-risk nonmetastatic breast cancer without diabetes, five years of adjuvant metformin did not improve invasive disease-free survival compared with placebo in hormone receptor-positive disease (hazard ratio 1.01, 95% CI 0.84 to 1.21, P = .93), and futility was declared for hormone receptor-negative disease.
- Clinical trial
In 167 non-diabetic patients with unresectable stage III non-small cell lung cancer, adding metformin to chemoradiation was well tolerated but did not improve 1-year progression-free survival (51.3% vs 60.4%, hazard ratio 1.15, P = .24) or overall survival.
- Clinical trial
In 151 non-diabetic patients randomized after polypectomy, one year of low-dose metformin was associated with a lower prevalence of total polyps (38.0% vs 56.5%, risk ratio 0.67) and adenomas (30.6% vs 51.6%) at follow-up colonoscopy compared with placebo.
- Observational
Metformin and reduced risk of cancer in diabetic patients
In a pilot case-control study of patients with type 2 diabetes in Tayside, Scotland, any exposure to metformin was associated with a lower odds of cancer (unadjusted odds ratio 0.79, 95% CI 0.67 to 0.93).
- Meta-analysis
Across 47 independent studies with 65,540 cancer cases in patients with diabetes, metformin use was associated with a 31% reduction in cancer incidence and a 34% reduction in cancer mortality, with smaller reductions in BMI-adjusted studies and studies without time-related biases and no consistent reductions for individual organ sites.
